Automatically assign player indexes to game controllers, and allow changing the player index for game controllers and joysticks.

Added the functions SDL_JoystickFromPlayerIndex(), SDL_JoystickSetPlayerIndex(), SDL_GameControllerFromPlayerIndex(), and SDL_GameControllerSetPlayerIndex()
