feat(defaults): visual CTRL-R for LSP mappings #28537

Problem:
The new LSP "refactor menu" keybinding "crr" is also defined in visual
mode, which overlaps with the builtin "c".

Solution:
Use CTRL-R instead of "crr" for visual mode.
