Problem: LspNotify autocmds were not being triggered for didChange
requests when being used during undo/redo (and possibly other) actions.
autocmds are blocked when calling on_lines() callbacks while doing the
undo/redo action.
Solution: Defer firing the autocmd until after the action is complete.
This is closer to what existed before, but now there's a check in the
deferred function to only fire the autocmd if the client is still
active and the buffer is still attached, if applicable.

Problem: LspNotify never passed a buffer when executing the autocmds, so
buffer-local LspNotify autocmd subscriptions didn't have the correct buf
in the event metadata. It was also wrapped in a schedule() so the actual
autocmd was delayed until after the event loop.
This could result in the wrong buffer receiving the notification if
multiple LspNotify autocmds with buffer filters were added. Only the
"latest" one would actually receive non-buffer-filtered autocmds, not
the matching one. It also caused listeners to receive the notification
"out of sync" with when the notification is actually sent. If a buffer
is being deleted (which fires a textDocument/didClose notification), the
notification is scheduled and fired after the buffer is already gone.
Solution: For LSP notifications that pertain to a particular buffer, set
it when executing the LspNotify autocmds so the callback functions that
are filtered on that buffer will get the correct notifications and the
metadata buf field will be correct. Additionally, there is no need to
wrap the LspNotify callback in vim.schedule when it can be called inline
when the notification to the rpc server is fired.
This is tested by removing now-unnecessary autocmds from semantic tokens
(InsertEnter and BufWinEnter should no longer be necessary now that
requests are fired by LspNotify). Without this fix, simply modifying a
buffer doesn't actually trigger LspNotify correctly, and the test for
that fails.

Problem: When multiline semantic token support was introduced, the loop
that finds the end line for a particular token didn't sanitize the token
length sent back by the LSP server. If the server returned an overflowed
length (near uint32 max), neovim would burn cpu and loop for an
extremely long time while trying to find the "end line" represented by
the massively large token, causing neovim to seemingly hang.
Solution: Stop looping once the calculated end_line reaches the actual
last line of the buffer.

Problem: The refactor to use Capability left around some cruft and
semi-broken configuration for debounce.
Solution: Clean up now-unnecessary helper methods and simplify
deprecated ones to pass through to the non-deprecated ones. `debounce`
now defaults to 200 for all buffers but is overridable via the
deprecated start() method, which continues to take the max value
specified for any client attached to the buffer.
If we wish to expose changing the debounce in a non-deprecated way, we
will need to consider a "configuration" function, or even a bespoke
method to set the debounce time on the main metaclass (or provide
options to override for a particular buffer). General configuration of
specific LSP features is an as-of-yet unsolved problem.

feat(lsp): use LspNotify for semantic tokens
Problem: The semantic token module is using its own debounce timer for
the buffer on_lines event. If its internal debounce is shorter than the
changetracking module's debounce, it's possible for semantic token
requests to fire for changed buffers before the textDocument/didChange
notification is sent to the server.
Solution: Trigger semantic token requests from the LspNotify autocmd
when the method is the didChange or didOpen notifications, which
enforces a strict happens-before relationship for the sync change
notification followed by a semantic token request.
Note: There is still an internal debounce mechanism in the semantic
token module to handle other debouncing needs specific to its
functionality, such as debouncing server refresh notifications and
handling WinScrolled events when using range requests.

Problem:
When a server sends workspace/codeLens/refresh while an automatic codelens
request is already scheduled, Nvim ignores the server refresh. This can leave
rendered codelens text stale until another buffer edit triggers a new request.
Solution:
Cancel the pending automatic request and send the server-requested refresh
immediately. This preserves request coalescing while giving explicit server
refreshes priority.

test(lsp): extract buf/util parts from lsp_spec.lua
Problem:
`test/functional/plugin/lsp_spec.lua` had grown into a large catch-all file that mixed core LSP client lifecycle coverage, `vim.lsp.buf.*` behavior, and `vim.lsp.util.*` behavior in one place.
Solution:
Split the large tests into more focused test files without changing test coverage or intended behavior.
After this change, `lsp_spec.lua` is more focused on core LSP client/config/dynamic-registration behavior.

Problem:
The snippet preview is not being highlighted by treesitter for
completion items from servers which don't support
`completionItem/resolve` (like gopls). This was broken by #38428.
Solution:
Call `update_popup_window` after updating the completion item with the
snippet preview.
I've added assertions to the `selecting an item triggers
completionItem/resolve + (snippet) preview` test case which covers the
snippet preview being shown since no tests failed when I removed the
`nvim__complete_set` call which actually populates the preview on this
codepath.
Problem:
Using nested `vim.Pos` objects to represent each `vim.Range` object
requires 3 tables for each `vim.Range`, which may be undesirable in
performance critical code. Using key-value tables performs worse than
using array-like tables (lists).
Solution:
Use array-like indices for the internal fields of both `vim.Pos` and
`vim.Range` objects. Use a metatable to allow users to access them like
if they were key-value tables.

The `vim.Pos` conversion interface for `extmark` indexing does not take
into account the difference in how a position on top of a newline is

`start` and `end` positions have different semantics, so they can't be
compared. `vim.Range` relies on comparing the `end` and `start` of two
ranges to decide which one is greater, which doesn't work as expected
because this of the different semantics.
For example, for the ranges:

implementation compares `b._end` (`col = 22`) and `a.start` (`col = 22`)
and concludes that, since `b.col` is not smaller than `a.col`, `b`
should be greater than `a`.
Solution:
- Use a `to_inclusive_pos` to normalize end positions inside of
`vim.Range` whenever a comparison between a start and an end position
is necessary.
Problem: clangd prepends a space/bullet indicator to label. With
labelDetailsSupport enabled, the signature moves to labelDetails,
making label shorter. This flips the length comparison in
get_completion_word, causing it to use item.label directly and
insert the indicator into the buffer.
Solution: only prefer filterText over label when label starts with non-keyword
character in get_completion_word fallback branch.
Problem: Generating snippet preview in get_doc() populated the
documentation field before resolve, so the resolve request was
never sent.
Solution: Move snippet preview logic into on_completechanged and
the resolve callback so it no longer blocks the resolve request.
Introduce _provider_foreach to iterate over all matching provider
capabilities for a given LSP method, handling both static and dynamic
registrations. Update diagnostic logic and tests to use the new
iteration approach, simplifying capability access and improving
consistency across features.

When code lens is enabled, `on_attach` is executed, but it does not trigger a redraw. Another event, eg, moving the cursor, is required to trigger a redraw and execute the decoration provider's `on_win`.

was never shown (compl_match_array == NULL) in ins_compl_stop().
When a match was inserted but the pum never displayed,
set the completed word so CompleteDone fires with reason "accept".
This keeps the "discard" reason intact when the user dismisses a visible
pum without confirming.
Problem:
In autocmd examples, using "args" as the event-object name is vague and
may be confused with a user-command.
Solution:
Use "ev" as the conventional event-object name.

If completeopt=popup is set, invoke completionItem/resolve when

Pull diagnostics (from `textDocument/diagnostic`) and push diagnostics
(from `textDocument/publishDiagnostics`) use the same namespace, which
is a problem when using language servers that publish two different sets
of diagnostics on push vs pull, like rust-analyzer (see
https://github.com/rust-lang/rust-analyzer/issues/18709#issuecomment-2551394047).
Solution:
Rename `is_pull` to `pull_id` which accepts a pull namespace instead of
just a boolean.
The wait added in #37853 doesn't seem to do anything as request is sent
immediately on InsertLeave, and the number 4 also seems wrong. Instead,
the actual cause for the flakiness that the feed() (and hence the buffer
change) may arrive before the scheduled initialization of capabilities,
causing there be only only one textDocument/semanticTokens/full request
instead of two.
Problem: Fake LSP server does not timeout or respond to SIGTERM as it
does not run the event loop.
Solution: Instead of io.read(), use stdioopen()'s on_stdin callback to
accumulate input and use vim.wait() to wait for input.
Also, in the test suite, don't stop a session when it's not running, as
calling uv.stop() outside uv.run() will instead cause the next uv.run()
to stop immediately, which cancels the next RPC request.
- Refactor LSP client to use unified provider-based capability lookup for
diagnostics and other features.
- Introduce `_provider_value_get` to abstract capability retrieval,
supporting both static and dynamic registrations.
