Problem:
A Visual-mode mapping that creates cursors (`xmap I Q0i`) replays its
commands only if cursors already existed when it started.

Solution:
Always collect `composite`. Consumers are still decided per frame
(`CmdFrame.consumers`) and at emit/cascade.
